Awareness of Commitment:

I am aware that as the parent it is my job to:

1.) Make sure the student practices regularly between lessons

2.) Make sure the student is prepared and on time at the lesson location each week

3.) Have an adult other than the instructor present at the lesson location for the entire duration of the lesson if the student is under 18 years of age.

4.) Cancel or reschedule a lesson with 24 hours notice (instructor’s discretion) for a lesson to be credited.  If 24 hours notice is not given (instructor’s discretion) the lesson will not be credited.

5.) Redeem any credited lessons within 3 months from the date of the credit.  Any credited lessons not redeemed within 2 months from the date of the credit will expire and that lesson will be forfeited.  Credited lessons can be used to add another lesson during the current month or be applied to the following month.

6.) Schedule lessons and make payments according to the Musical Way Academy Scheduling and Payment Plan.
